Frequency of uropathogens and isolation of extended-spectrum beta lactamases (ESBLs) secreting uropathogenic Escherichia coli (UPEC) at Hyderabad region

Abstract
A common infection that primarily affects women is urinary tract infection (UTI). The infection commonly caused by Gram-negative bacteria especially Escherichia coli (E. coli). Antibiotic resistance steadily rises to third-generation cephalosporins, leading to the emergence of β-lactamases, particularly Extended spectrum β-lactamases (ESBLs) and multidrug resistance (MDR) isolates, which is a concerning situation that leads to treatment failure. Hence, the antibiotic sensitivity pattern thus established will improve the treatment of infection and prevent the chances of therapeutic failure with irrational use of antibiotics. This cross-sectional study is being, piloted from February 2021 to June2021 at Hyderabad. Six hundred (600) midstream urine samples collected aseptically; inoculated on selected media and incubated for 24-48 hours. The organisms were, then identified by the series of biochemical test. Antibiotics sensitivity test and ESBL detection were done as per criteria. 52 (40%) isolates out of 600 samples were found to be E. coli followed by Klebsiella spp, Enterobacter spp, Pseudomonas spp and Proteus spp were 19 (14.61%), 13 (10%), 10 (7.69%) and 9 (6.92%) respectively. Out of 52 UPECs 19 (36.53%) were ESBL producers and 33 (63.46%) were non-ESBL producers, out of which 17 (89.47%), 13 (68.42%) and 16 (84.21%) isolates were sensitive to meropenem, fosfomycin and amikacin respectively. However, in 11 (57.89%) isolates resistance was observed to ciprofloxacin. In our community, ESBL-producing UPEC is highly prevalent; yet, the resistance to meropenem, fosfomycin, and amikacin, is still in the low occurrence, and are the choice of drug to treat ESBLs producing UPEC.
